Optimizing Medication Management in Primary Care: A Medical-Social Approach Guided by Evidence-Based Policy

To promote the professional development of primary care services in Hong Kong community pharmacy, it is our pleasure to invite Professor Ian Wong from the Department of Pharmacology and Pharmacy from the University of Hong Kong to share the current development of the community pharmacy in Hong Kong. ✨ About the Seminar ✨
This seminar provides a comprehensive overview of medication management services in primary care and highlight evidence-informed practices in Australia. Professor Chen will share his experience and recommendations on evidence-informed policy changes including the integration of medication management services in primary care settings, government subsidization and service sustainability. Meanwhile, medical-social collaboration is also of uttermost importance in medication management services, social determinants of health (e.g. socioeconomic status, cultural beliefs and access to resources), strategies of medical-social collaboration and referral mechanisms for medication management services will also be shared in this seminar.

Jointly organized with Primary Care Pharmacy Consortium, this Seminar will serve as one of the Continuing Pharmacy Education (CPE) Programmes and CPE points will likely be awarded after attending the Seminar.
